What is DDH?
Developmental Dysplasia of the Hip (DDH) is a congenital condition in which the hip joint fails to develop normally, leading to instability or dislocation of the femoral head from the acetabulum. It ranges from mild laxity to complete dislocation. In India, DDH is a significant public health concern due to its relatively high prevalence, especially in communities where swaddling with extended legs is practiced. Early detection through newborn screening using the Barlow and Ortolani maneuvers, followed by hip ultrasound, is critical to prevent long-term disability. The condition is most commonly diagnosed in infancy and early childhood during routine paediatric check-ups or when a child presents with a limp or leg length discrepancy. DDH is a frequent topic in Indian medical entrance exams such as NEET PG, AIIMS, and state PG tests, where questions often focus on risk factors (breech presentation, family history, female sex), diagnostic methods, and management options including the Pavlik harness. Awareness campaigns by the Indian Academy of Pediatrics emphasize universal screening to reduce the burden of late-diagnosed cases. Timely intervention ensures excellent outcomes, while delayed treatment may require surgical reduction and increase the risk of avascular necrosis.
