What is NPH?
Normal Pressure Hydrocephalus (NPH) is a neurological disorder characterised by an abnormal accumulation of cerebrospinal fluid in the brain's ventricles, despite normal intracranial pressure. It primarily affects older adults and is often misdiagnosed as Alzheimer's disease or Parkinson's disease due to overlapping symptoms. The classic triad includes gait disturbance, urinary incontinence, and cognitive decline — collectively known as the 'Wet, Wacky, and Wobbly' syndrome. In India, NPH is increasingly recognised as a treatable cause of dementia, especially in geriatric neurology clinics and tertiary care hospitals. Diagnosis involves brain imaging (CT or MRI) showing enlarged ventricles, lumbar puncture to measure CSF pressure, and sometimes a tap test to assess improvement after fluid drainage. Treatment typically involves surgical placement of a ventriculoperitoneal shunt to divert excess CSF. For medical students and neurologists in India, NPH is a key topic in the neuro-psychiatry curriculum and appears in postgraduate entrance exams such as NEET PG.
