What is VDZ?
Vedolizumab, abbreviated as VDZ, is a humanised monoclonal antibody classified as a gut-selective anti-integrin biologic therapy. It works by blocking the alpha-4 beta-7 integrin on the surface of circulating white blood cells, preventing them from binding to MAdCAM-1 molecules in the intestinal lining and thereby reducing localised inflammation in the gut. In India, VDZ is prescribed by consultant gastroenterologists for adult patients suffering from moderate to severe ulcerative colitis and Crohn's disease who have not responded adequately to conventional treatments such as corticosteroids, immunomodulators, or anti-TNF agents like infliximab and adalimumab. The drug is administered through intravenous infusion, typically at weeks zero, two, and six, followed by maintenance doses every eight weeks at specialised infusion centres across metropolitan cities like Mumbai, Delhi, Bengaluru, and Chennai. It represents a significant advance in the management of inflammatory bowel disease, offering a targeted approach with a favourable safety profile owing to its gut-specific mechanism.
