What is UMN?
Upper Motor Neuron (UMN) refers to the nerve cells that originate in the brain's motor cortex and travel through the brainstem and spinal cord, carrying signals that control voluntary movement in the body. These neurons form a critical part of the central nervous system and are responsible for initiating, regulating, and maintaining muscle tone and posture. In India, UMN lesions are extensively studied in neurology, orthopaedics, and rehabilitation medicine, as they commonly result from strokes, spinal cord injuries, multiple sclerosis, or traumatic brain injuries. Medical students across Indian colleges, particularly those preparing for NEET PG, AIIMS, and JIPMER examinations, must understand the clinical signs of UMN damage, which include spasticity, hyperreflexia, positive Babinski sign, and weakness without significant muscle wasting. Distinguishing UMN lesions from lower motor neuron lesions is a fundamental clinical skill taught in MBBS curricula and tested routinely in postgraduate entrance exams. Physiotherapists and rehabilitation specialists in India also frequently assess UMN function when designing treatment plans for patients with hemiplegia, paraplegia, or cerebral palsy.
