What is RHF?
Right Heart Failure (RHF) is a clinical condition where the right ventricle of the heart fails to pump blood effectively into the pulmonary circulation. This leads to a backlog of blood in the venous system, causing symptoms such as peripheral edema, jugular venous distension, hepatomegaly, and ascites. The most common cause of RHF is left heart failure, which increases pulmonary pressure and strains the right ventricle. Other causes include pulmonary hypertension, chronic obstructive pulmonary disease (COPD), pulmonary embolism, and valvular heart diseases like tricuspid regurgitation. In India, RHF is particularly relevant due to the high burden of rheumatic heart disease (RHD), which commonly affects the mitral valve and eventually leads to pulmonary hypertension and right heart strain. The condition is frequently encountered in cardiology and internal medicine wards across hospitals. Management focuses on treating the underlying cause, reducing fluid overload with diuretics, and using medications like inotropes or vasodilators when indicated. RHF is a key topic in undergraduate and postgraduate medical examinations in India, including NEET PG and AIIMS, where questions often link it to specific etiologies like COPD or RHD. Early recognition and appropriate referral can significantly improve patient outcomes.
