What is PTH?
Parathyroid hormone (PTH) is a key regulator of calcium and phosphate metabolism in the human body. It is secreted by the parathyroid glands, four small endocrine glands located behind the thyroid gland in the neck. PTH acts on bones, kidneys, and the intestines to increase blood calcium levels when they fall too low. In the skeleton, it stimulates osteoclasts to resorb bone, releasing calcium and phosphate. In the kidneys, it enhances calcium reabsorption and phosphate excretion, while also activating vitamin D to boost intestinal calcium absorption. In India, PTH levels are commonly measured in patients with kidney disease, osteoporosis, and parathyroid gland disorders such as hyperparathyroidism or hypoparathyroidism. The test is often ordered alongside serum calcium, vitamin D, and other markers. PTH is especially relevant for Indian patients with chronic kidney disease, which has a high prevalence due to diabetes and hypertension. Accurate interpretation of PTH helps guide treatment decisions, including calcium and vitamin D supplementation or parathyroidectomy. For medical exams like NEET PG or USMLE, PTH regulation and its feedback loop with calcium and calcitriol are frequently tested topics.
