What is PFO?
Patent Foramen Ovale (PFO) is a congenital heart condition where a small flap-like opening in the interatrial septum fails to close after birth. In fetal circulation, the foramen ovale allows oxygenated blood to bypass the lungs, but it typically seals within the first year. When it remains open, it is called a PFO. In India, PFO is often detected incidentally during echocardiography for unrelated symptoms like migraine or stroke. It is a common finding, affecting about 25% of the population, and is usually asymptomatic. However, it can allow blood clots to cross from the right to the left atrium, leading to paradoxical embolism and cryptogenic stroke, especially in young adults. Diagnosis is primarily through transthoracic or transesophageal echocardiography with bubble study. Management includes antiplatelet therapy or percutaneous closure in selected high-risk patients. For medical students preparing for NEET PG or USMLE, PFO is frequently tested as a cause of cryptogenic stroke and right-to-left shunt. Knowledge of its anatomy, clinical implications, and treatment options is essential for cardiology and neurology exams.
