What is NEXUS?
The National Emergency X-Radiography Utilization Study (NEXUS) is a clinical decision rule developed to help emergency physicians identify patients with a low probability of cervical spine injury, thereby reducing the need for unnecessary imaging. In India, NEXUS is widely adopted in hospital emergency departments, particularly in trauma care, to guide the use of X-rays and CT scans. It was originally derived from a large multicenter study in the United States but has been validated in Indian populations as well. The criteria include five factors: absence of midline cervical tenderness, no focal neurological deficit, normal level of alertness, no intoxication, and no painful distracting injury. When all five are met, the patient is considered low-risk and imaging may be safely avoided. Medical students and emergency medicine residents in India study NEXUS as part of their MBBS and postgraduate curriculum, as it appears in examinations like NEET PG and FMGE. The tool is used daily in casualty wards across the country to streamline trauma management and reduce radiation exposure and healthcare costs.
