What is ILD?
Interstitial Lung Disease (ILD) refers to a broad group of lung disorders that involve inflammation and scarring (fibrosis) of the interstitium—the tissue and space around the air sacs of the lungs. This scarring thickens the lung tissue, making it difficult for oxygen to pass into the bloodstream. In India, ILD is increasingly recognized due to rising exposure to environmental pollutants, occupational hazards like silica and asbestos, and conditions such as connective tissue diseases and tuberculosis. It is typically diagnosed through high-resolution CT scans and pulmonary function tests, and management includes anti-inflammatory medications, antifibrotic drugs, and oxygen therapy. Pulmonologists and respiratory therapists frequently use the term in clinical settings and medical education. ILD is a significant topic in Indian medical exams such as NEET PG, USMLE equivalents, and MBBS final-year examinations, where students are tested on distinguishing ILD from other interstitial lung diseases based on etiology and histopathology. Awareness is crucial in India where misdiagnosis as tuberculosis often occurs due to overlapping symptoms like chronic cough and breathlessness. Multidisciplinary care involving pulmonologists, radiologists, and rheumatologists is advancing in major cities, but rural access remains limited.
