Balanitis Xerotica Obliterans (BXO) is a chronic, inflammatory skin condition that primarily affects the foreskin and glans of the penis. It is considered a form of lichen sclerosus localized to the male genitalia. The condition is characterized by whitish, atrophic patches that can lead to phimosis (tightening of the foreskin), meatal stenosis, and eventually urinary obstruction if left untreated. In India, BXO is a common cause of pathological phimosis in both children and adults, and is frequently encountered in urology and dermatology clinics. It is often misdiagnosed as recurrent balanitis or simple phimosis, highlighting the need for proper clinical and histopathological evaluation. BXO is not sexually transmitted, though its exact etiology remains unclear; autoimmune factors are suspected. Treatment typically involves topical corticosteroids, but advanced cases may require circumcision, meatotomy, or urethral dilatation.
