What is VKA?
Vitamin K Antagonist, commonly abbreviated as VKA, refers to a class of oral anticoagulant medications that work by blocking the action of vitamin K in the liver, thereby preventing the synthesis of clotting factors II, VII, IX, and X. The most widely prescribed VKA across India is warfarin, although acenocoumarol is also extensively used in Indian clinical practice due to its shorter half-life and relatively predictable response. VKAs are routinely prescribed in India for managing deep vein thrombosis, pulmonary embolism, atrial fibrillation, and for patients who have received mechanical heart valve replacements. Indian patients generally require lower maintenance doses of VKAs compared to Western populations, owing to genetic polymorphisms affecting drug metabolism, lower average body weight, and dietary patterns rich in green leafy vegetables. Regular monitoring of the International Normalised Ratio is essential to maintain therapeutic anticoagulation and avoid bleeding complications. VKAs hold strong relevance in Indian medical examinations such as NEET-PG, AIIMS, and PGI, where questions frequently focus on their pharmacology, monitoring, drug interactions, and bridging strategies with newer agents.
