What is TJI?
Total Joint Infection, commonly abbreviated as TJI, refers to a serious post-operative or hematogenous infection affecting an entire prosthetic joint replacement, such as a hip, knee, or shoulder arthroplasty. It is one of the most feared complications in orthopedic surgery, often caused by bacteria like Staphylococcus aureus or Staphylococcus epidermidis that colonize the implant surface and form resistant biofilms. In India, where the volume of joint replacement surgeries has grown rapidly due to rising cases of osteoarthritis, road accidents, and an aging population, TJI is a major clinical and economic concern, frequently discussed in orthopedic conferences and continuing medical education programs. Management typically involves prolonged antibiotic therapy, surgical debridement, and sometimes complete revision of the prosthesis in two stages. TJI is a frequently tested topic in postgraduate medical entrance examinations including NEET-SS, AIIMS, and PGI entrance tests, as it tests knowledge of microbiology, pharmacology, and surgical decision-making.
