What is NUG?

Necrotizing Ulcerative Gingivitis (NUG) is an acute, painful infection of the gums characterised by necrosis, ulceration, and bleeding of the interdental papillae. Often referred to as trench mouth, it is caused by an overgrowth of anaerobic bacteria, typically Fusobacterium nucleatum and spirochetes, and is associated with stress, poor oral hygiene, smoking, and immunosuppression. In India, NUG is seen in both urban and rural dental clinics, particularly among young adults and individuals with compromised immunity. It is commonly taught in dental and medical curricula as a classic periodontal emergency. Diagnosis is primarily clinical, based on the sudden onset of foul odour, pain, and pseudomembrane formation. Treatment involves gentle debridement, antibiotics (usually metronidazole), and improved oral hygiene. NUG is relevant for Indian dental students preparing for NEET MDS and AIPGD exams, as questions frequently cover its aetiology, clinical features, and management. Public health awareness in India stresses early intervention to prevent progression to necrotising periodontitis. The condition is also noted in military and camp settings due to crowding and stress.
