What is EPH?
Epidural Hematoma (EPH) is a life-threatening medical condition characterized by the accumulation of blood between the inner table of the skull and the dura mater, the outermost layer of the brain's protective coverings. It most commonly results from traumatic brain injury, such as falls or vehicular accidents—a leading cause in India due to high traffic density and inadequate road safety measures. The bleeding is typically arterial, often from the middle meningeal artery, leading to rapid expansion and increased intracranial pressure. If not treated promptly, EPH can cause brain herniation and death. In Indian emergency rooms, timely diagnosis via CT scan is crucial. The classic lucid interval—where the patient briefly regains consciousness before deteriorating—is a hallmark. Surgical evacuation through a craniotomy or burr hole is the definitive management. EPH is a key topic in Indian medical entrance exams like NEET PG and AIIMS, as well as in undergraduate MBBS curricula. Understanding its pathophysiology, signs such as asymmetric pupils and hemiparesis, and emergency intervention is vital for medical professionals in India's trauma care system.
