What is BZD?
Benzodiazepines (BZDs) are a class of psychoactive drugs primarily used for their sedative, anxiolytic, muscle relaxant, and anticonvulsant properties. In the Indian healthcare system, BZDs are commonly prescribed for short-term management of anxiety disorders, insomnia, acute seizure episodes, and pre-operative sedation. They work by enhancing the effect of the neurotransmitter GABA at the GABA-A receptor, leading to a calming effect on the brain. Despite their therapeutic benefits, BZDs carry a risk of dependence, tolerance, and withdrawal, which has led to strict prescription regulations under India’s Narcotic Drugs and Psychotropic Substances Act. In clinical practice, drugs like diazepam, lorazepam, and alprazolam fall under this category. They are administered orally, intravenously, or intramuscularly depending on the urgency and condition. BZDs are also used in emergency settings for alcohol withdrawal management and status epilepticus. However, long-term use is discouraged due to potential cognitive impairment and addiction. Medical students and pharmacology aspirants must understand BZD pharmacokinetics, side effects (drowsiness, ataxia, amnesia), and contraindications. In exams like NEET PG or FMGE, questions on BZD mechanism, antidote (flumazenil), and interactions are frequently asked. Overall, BZDs remain a vital but tightly controlled tool in India’s mental health and emergency care landscape.
